Social Work Services - Summerfield Health Care Center - Cloverdale, IN

Summerfield Health Care Center is seeking a social worker to coordinate the in-house psychological and social services provided to assure residents attain the highest practical level of independence. Candidate will also serve as community liaison and educator supporting families and groups affected by Huntington’s Disease and those referral agencies serving individuals with HD.

Qualifications:

• Bachelor’s Degree in Social Work or related field required.


• One to Two years’ experience in social services, preferably in long-term care.


• Strong customer service values a must.
