Community Health Worker - Valley Professionals Community Health Center - Clinton, IN

Valley Professionals Community Health Center is looking to hire a Community Health Worker for its Clinton office.

The Community Health Worker is responsible for helping patients navigate and access community services, as well as, assisting patients in applying for publicly sponsored health care insurance plans. Must be willing to work flexible hours, including some weekends and evenings.

High school diploma required. 3 or more years of experience in a public or private health or social agency is preferred.
