Medical Records Clerk - Union Hospital - Terre Haute, IN

High School/GED

Prior experience in a healthcare facility


ESSENTIAL JOB FUNCTIONS:


  • Pulls and files patient medical records.

  • Files correspondence, Hospital, lab, etc.

  • Delivers patient records to specific areas/departments correspondence, reports.

  • Maintains incoming and outgoing fax.

  • Collects records, reports from out boxes, returns them to medical records.

  • Distributes mail following courier runs.

  • Stamps and sorts mail for evening pickup.

  • Prepares new patient charts.

  • Repairs old patient charts as needed.

  • Assures proper ICD-9 code is listed in patient chart by checking diagnosis listed or asking doctor.

  • Completes disability or leave forms.

  • Maintains medical record release requests.

  • Processes death certificates.

  • Purges inactive patient records.

  • Performs other related duties as assigned by manager.

  • Promptly responds to customers in a friendly manner, listening carefully and using customer’s name. Every customer greeted with a smile.

  • Answers telephone promptly, consistently identifies self and department and returns calls promptly.

  • Is responsible for ensuring that all patient, facility and employee information is not compromised.

  • Respects privacy of all by eliminating gossip and unprofessional talk.

  • Displays a professional appearance at all times, including appropriate dress as defined in employee handbook and wearing visible identification badge at all times.

TRAINING, EXPERIENCE, SKILLS AND ABILITIES NEEDED:
1. Ability to work in isolated area for long periods of time.


2. Ability to work unsupervised. Self-starter.


3. Working grasp of standard written and verbal English; accomplished in alphabet as well as numeric filing.


4. Must meet and interact successfully with a wide variety of people.


5. Working knowledge of ICD-9 codes.


6. General knowledge of medical terminology.


7. Must possess good telephone skills.


PHYSICAL DEMANDS:
While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand, walk, sit, use hands to finger, handle, or feel objects, tools, or controls, reach with hands and arms, stoop, and kneel. The employee is occasionally required to climb or balance.


The employee must frequently lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, color vision, and the ability to adjust focus.


OSHA III Classification.


TOOLS AND EQUIPMENT:
1. Computer, ten-key calculator, typewriter, fax, postage machine, copier, various printers, and other general office equipment.
