JOB SUMMARY


The Clinical Coordinator is responsible for answering inquiries, and disseminating information about the agency. This position establishes and maintains effective and efficient systems for paper flow and communications. This position assists with daily operations of the agency including coordinating and maintaining communication between Clinical Supervisors and the agency.


JOB DUTIES


  • Maintains organized, effective and efficient systems and communication to ensure the continuity of quality patient care delivered.

  • Assists in preparing dashboards as requested by director.

  • Assists in evaluating agency operations and systems and identifies ways to enhance workflow and productivity.

  • Assists staff in maintaining current and accurate medical records and utilization review as necessary.

  • Assists clinical supervisor with non-clinical phone inquiries.

  • Notifies supervisor of any problems requiring administrative attention and intervention.

  • Distributes, collects, maintains inventory and cleans HomMed equipment.

  • Maintains a variety of tracking systems.

  • Processes in and out mail daily for the department.

  • Responsible for all filing in required binders for department accuracy.

Required Skills
PHYSICAL AND ENVIRONMENTAL DEMANDS

  • Complete required work on a daily basis.

  • Cope with work involving ill, disabled, and dying patients.

  • Sit, stand, bend, lift, and move intermittently.

  • Move equipment and supplies necessary for job functions.

  • Read, speak and write English in order to fulfill job functions in an understandable way.

  • See/hear or use prosthetics that enable accomplishment of job.

  • Meet the health requirements of the agency.

  • Abide by all Physical safety and bloodborne pathogen protocol to minimize illness and/or injury.

  • Move, support and/or transfer patients as required to perform essential functions of position.


Required Experience


  • EDUCATION: High School Graduate or GED equivalent. Associate Degree preferred.

  • LICENSURE: Must have current and valid driver’s license.

  • EXPERIENCE: 1-year experience in a health care setting.

  • SKILLS: Requires interpersonal skills and ability to communicate effectively with staff and directs patient referrals appropriately. Knowledge of medical terminology required, Medicare and Medicaid guidelines preferred. Able to demonstrate problem-solving skills, organizational skills and attention to detail. Must have strong verbal and written communication skills. Ability to operate computer equipment, FAX and photocopy machines necessary.

  • TRANSPORTATION: Reliable transportation and valid automobile insurance coverage.

  • HEALTH STATUS: Meets all applicable agency policies and procedures related to health screening and required testing.
