Position Summary


Responsible for a leadership role in coordination of integrated primary and behavioral healthcare services to individuals with serious mental illness using evidence-based interventions. Performs individual, family and community education, coordinates client appointments and case management / skill training, promotes consumer advocacy, initiates community systems collaboration and referral facilitation, coordinates crisis management education and quality assurance follow-up. Assist clients by locating, coordinating, and monitoring consumer focused and goal oriented treatment, services, activities and interventions that are appropriate and accessible. The position monitors access to care and work with consumers in multiple settings based in office and community.

Provides collaboration with participants, physical and behavioral health care providers to improve participate health status and to assist participants in identifying and overcoming barriers that interfere with achieving health-related goals.


Conducts community outreach and education to raise awareness about the health insurance coverage exchange to assist consumers in enrollment options.


Provide education, reinforcement and frequent communication with consumers to encourage, support and assist them in managing and improving their primary and behavioral health care


Have knowledge of Hamilton Center’s programs and services by geographic location and division – constantly keep updated on changes in services


Have a clear understanding of Hamilton Center’s target market(s)


Initiate events designed to attract and educate referral sources and consumers


Initiate tours, public speaking and other outreach opportunities to promote services and programs


Consistently exercises discretion and judgment to analyze, interpret, make deductions and then decide what actions are necessary based on the varying facts and circumstances of each individual and family situation.


Works with clinical supervisory assistance and evaluate possible courses of conduct and making decisions where there is no opportunity to seek supervisory assistance.


Maintains an active caseload providing interventions as needed and within area of expertise and credentials; assures procurement of additional services as needed


Acquires and provides venues for client and family wellness and provides detailed information to concerned providers regarding an assigned client to establish the foundation for the Treatment Plan


Works with the family on a day-to-day basis using professional judgment and discretion to implement the team determined Treatment Plan


Assists in development, implementation and revision of individual treatment plans; assures that services provided are specified in the Treatment Plan and monitors progress toward treatment goals


Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals


Coordinates crisis management for individuals and families; makes referrals and considers interventions as appropriate


Maintains client and program records in accordance with applicable standards and regulations according to Hamilton Center policy and procedures.


Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature


Participates in continuing education activities, remaining knowledgeable in area(s) of expertise


Attend meetings as appropriate and meet regularly with supervisor to exchange pertinent information and receive supervision


Completion of OBHP modules within one year of hire.


Completion of required CANS and/or ANSA training and ongoing certification


Assists with coordination of transport of enrollees to attend appointments


Performs other duties as assigned


Minimum Qualifications/Requirements


Bachelor’s degree in Nursing (BSN), social work, sociology, psychology, education or counseling required.


A minimum of one year post grad experience in a social service setting.


Knowledge and skills in community based health care and case management preferred


Good interpersonal and communication skills, ability to problem-solve with genuine interest in helping adults with mental illness access behavioral or primary care.


Bilingual in English and Spanish a plus.


Must value cultural differences and have the ability to work with consumers in multiple office and community settings


Ability to demonstrate competent use of Electronic Medical Records


Ability to utilize Microsoft Office applications and the internet


Flexible scheduling required


Certificates, Licenses, Registrations


Valid driver’s license in accordance with HCI Motor Vehicle Policy (Valid Chauffeurs License if drive 15-passenger vans with clients)


Maintain current Crisis Prevention Intervention (CPI) certification


Maintain CPR and other certification as required per HCI policy.


Pursue training as a state navigator


Physical Demands


While performing the duties of the job, the employee is regularly required to communicate in person or by telephone. The employee must be able to travel to client locations. The employee is frequently required to be physically mobile to perform her duties in community.


Conditions of Employment


These characteristics are representative of those an employee may encounter performing the essential functions of the job. The employee works with seriously mentally unwell individuals where a high degree of stress is possible. The position does not normally involve exposure to blood, body fluids or tissues; but it may require performing unplanned tasks where exposure may occur. The position requires the use of standard precautions. This is in particular implied for Master in nursing degrees.


Satisfactory reference and background investigation checks


Completion of a pre-employment drug screening and completion of post-employment drug or alcohol tests upon reasonable suspicion of use


Demonstrated computer literacy through successful completion of pre-employment testing may be required.


Satisfactory completion of an Indiana Department of Child Services criminal fingerprinting background, Indiana State Criminal History, Sex and Violent Offender Registry, Child Protection Services History, and local law enforcement agency/county sheriff checks at the time of hire and every four years (or at contract renewal when applicable) for any employee that has direct contact on a regular and continuing basis with DCS and IVB clients.


Completion of tuberculin screening no later than three days prior to the new employee’s first work day and annually thereafter


Completion of Center-wide new employee orientation and ALL required paperwork prior to reporting to work


Completion of HCI Commitment to Quality Training, Trauma Informed Care, and Relias Learning


Completion of HCI Case Management Training


Attendance at all mandatory staff development and training


Successful completion of a six month on-the-job orientation period


Participation in payroll electronic deposit


Adherence to Compliance Program Plan
