Outreach Worker-PATH (Projects for Assistance in Transition from Homelessness) Grant

Division: MED/24 Hour Services/Outpatient

Position Summary


Provide a wide range of services to clients struggling with homelessness or risk of homelessness. Coordinate outreach efforts to local shelters and other agencies to assist in referral to services and housing. Work with clients in outpatient, inpatient and subacute/group home living services as needed.

Essential Duties/Responsibilities

Conduct outreach efforts to homeless population in the community by providing referrals and assistance to access behavioral health, substance abuse services, housing and other related needs.


Assist in after care and discharge planning when needed from inpatient unit to outpatient services.


Be knowledgeable and informed on a variety of community resources including housing assistance, outpatient services, Medicaid and Medicare, SNAP, etc.


Be knowledgeable and develop relationships with local agencies that assist with rapid re-housing and housing those that are chronically homeless.


Develop, implement and modify, as needed, processes for improvement.


Provide crisis management for clients, including on-call; make recommendations for interventions as appropriate.


Follow Center procedures regarding the provision of client care and documentation.


Maintain client and program records in accordance with applicable standards and regulations with PATH grant requirements through DMHA.


Participate in continuing education activities, remaining knowledgeable in area(s) of expertise, and meet requirements for professional staff development.


Attend meetings and staffing as appropriate and meet regularly with supervisor to exchange pertinent information and receive supervision.


Serve as a resource person to Center and community in area(s) of expertise, including presenting classes, speaking engagements and agency consultation.


Maintain a high level of ethical conduct regarding confidentiality, dual-relationships and professional stature.


Ability to demonstrate competent use of Profiler


Perform any other duties as assigned.


Completion of required CANS and/or ANSA training and ongoing certification


Understand Trauma Informed Care and implement in practice.


Become trained in Housing First Model and SOAR (SSI/SSDI Outreach Access and Recovery).


Perform the above listed duties with or without reasonable accommodations.


Minimum Qualifications/Requirements


Bachelor degree in Social Work or related.


Advanced knowledge and skills regarding mental health, addiction and homelessness.


Knowledge of the natural history of dependence and the relevant biological and sociocultural factors.


Good written and oral communication skills.


Good interpersonal skills.


Demonstrate competence in assessment of and treatment planning for adults.


Understanding and knowledge of the range of treatment needed and appropriate utilization of available treatment resources.


Willingness to use reliable personal transportation in work capacity.
