JOB DESCRIPTION


JOB TITLE: Home Care Registered Nurse (RN)


DIVISION: Health Care


REPORTS TO: Clinical Services Supervisor


 


Summary:


Teach, arrange for, supervise and/or provide nursing care of the sick and
injured in the 
home.  Plan coordinated nursing services for patients under
care.  Appraise patient
s health needs and hazards, existing or potential. 
Refers clients to appropriate personnel
or services.  All treatment and
diagnostic procedures are carried out under medical
direction.  Perform
duties in accordance with the Indiana Nurse Practice Act (IC 25-23).


 


Salary Level:


Commensurate with Education and Experience


 


Essential Duties And Responsibilities include the following, except where
services are limited to therapy only.  Other duties may be assigned.


1.   Make the initial evaluation visits


2.   Perform initial and ongoing comprehensive assessments of the patients
      needs, including OASIS
 (Outcome and Assessment Information Set) 
      assessments at appropriate time points.


3.   Regularly reevaluate the patients nursing needs


4.   Initiate the plan of care and make necessary revisions


5.   Initiate appropriate preventive and rehabilitative nursing procedures


6.   Prepare clinical and progress notes


7.   Coordinate services


8.   Refer to other services as needed, communicating pertinent information


9.   Inform the physician and other appropriate medical personnel of changes 
      in the patients 
condition and needs.


10. Evaluate the effectiveness and outcomes of care.


11. Plan for discharge from service


12. Counsel, educate and supervise the patient and family in meeting
      nursing and 
related needs, regarding the disease process, self-care 
      techniques, health promotion a
nd disease prevention strategies.


13. Promotion of patient/family independence.


14. Participate in in-service programs


15. Supervise and teach other nursing personnel, including LPNs and
      paraprofessionals.


16. Delegate and direct the duties, tasks and activities of LPNs and other 
      individuals, as
appropriate.


17. Accept and carry out physician, chiropractor, podiatrist, dentist and
      optometrist orders 
(oral and written).


18. Assist the physician, chiropractor, podiatrist, dentist or optometrist in 
      evaluating level of 
function.
19. Provide those services and/or treatments requiring substantial and
      specialized nursing skill




20. Give skilled care and treatments to patients requiring intermittent
       nursing service as ordered


21. Participate in RN on-call rotation


22. Provide visits to Hospice patients as needed


23.All other duties as assigned by supervisor.


 


Qualification Requirements:


To perform this job successfully, an individual must be able to perform

Certificates, Licenses, Registrations:


1.   Graduate of an NLN accredited program, preferably with a 
      baccalaureate in nursing (BSN) or 
associates degree in nursing (ADN)
      with 2 years clinical experience.


2.   Valid RN licensure by the State of Indiana


3.   Prefer experience in home health care field


4.   Must provide dependable automobile, proof of current insurance
      coverage and able to drive in 
inclement weather.


5.   Have a valid Indiana drivers license
