Clinical Care Coordinator

The Clinical Care Coordinator will provide nursing care assistance to those Sisters of Providence who reside independently either on- or off-campus.  The Clinical Care Coordinator will consult and advise the sisters regarding healthcare needs and services, assist sisters in understanding their own medical needs, and perform all essential functions of care coordination.  On occasion, the Clinical Care Coordinator may be required to travel to accommodate a sister living elsewhere in the United States.

QUALIFICATIONS

BSN preferred from accredited nursing program.

5-8 years of relevant experience.

Home health care management experience or equivalent preferred.

Geriatric Care Manager (GCM) certification preferred.
