Data Analyst
for our
Case Management Department
. Qualified candidates will have Health Information/Medical Records experience or at minimum prior experience in hospital utilization and insurance review.

Position Responsibilities:


  • Assists Case Managers with overflow insurance reviews, contacts and documentation as indicated by census needs.

  • Assists Case Managers with delivering Important Message Letters to patients per CMS standards

  • Collaborates with the RN case manager or Social Work case manager to identify patients needing post hospital vendor services.

  • Facilitate communication with the post hospital vendor to provide the needed information to support the handoff communication via electronic notification using Curaspan/Discharge Central.

  • Participates in interdisciplinary huddles to facilitate effective patient throughput/planning.

  • Completes required forms and informs them of the alternative available to meet the patient needs.

  • As directed by the Case Manager, refers patients who require extended care post discharge to post-acute care facilities. Once an appropriate bed is located, arranges for the patient’s transfer.

  • Collaborates all care planning with the RN case manager or Social Work staff to ensure no delays with patient transitions.

  • Documents interventions and discharge plans in the patient’s medical record congruent with departmental policies.

  • Maintain customer focus exceptional care by using AIDET and other department initiatives.

  • Communicates discharge plans with the patient and/or family, physician, nursing and interdisciplinary team.

  • Performs other duties as assigned to meet the needs of the department.

  • Prepares agenda, schedules room, dietary requests, prepares packets, type’s correspondence, maintains binder of PI projects, Avoidable days, UR data entry, Case Management updates/meetings

  • Serves as a receptionist, answers telephone and takes messages, photocopying, routes mail and all other correspondence

  • Maintains binder of all department archived policies

  • Responsible for maintaining, restocking and requisitioning department supplies and equipment. Be proficient on E-Way by Corporate Express

  • Understands and satisfies the needs of the patient population appropriate to the age of the patient served (adults, geriatric, adolescent, pediatric, and parents of neonates)

Qualifi

cations

Essential educational/licensure, skills and experience:


  • High school graduate with computer skills.

  • Must have knowledge of medical terminology.

  • Prior experience in hospital utilization and insurance review required.

  • Current BLS certification maintained throughout employment with Terre Haute Regional Hospital

Valued but not required educational/licensure, skills and experience:

  • Previous experience in Medical Records or graduate of an approved accredited Medical Records Technician program preferred.

  • Dependable and good attendance.

  • Ability to work and communicate with all employees.
